Forms and Documents for Parents

Athletic Forms:
In order for any student to participate in an athletic activity, there are certain forms that are required to be submitted before a student is able to participate.  Please find the required forms below, and return them to Lindsey Peck the High School office, or email your scanned documents to chris.coffman@grinnell-k12.org.

If your student is participating in sports please return all 5 forms
 
Travel Forms:
The Grinnell-Newburg School District takes the safety of our students very seriously.  In order for any student to participate in any school activity, students must use school-provided transportation (or ride home with a parent/guardian).  In the event that a student wishes to travel with someone other than a parent/guardian or school-provided transportation, parents must submit a non-parent travel form.  Please return to Lindsey Peck.

Health Forms:

Please download the following health forms for your student's needs.  Proper form submission is required in order for our staff to best serve you. 

If you student requires medication, please submit the Medication Administration Form.

Bullying
Please fill out and submit the following document to report an incident of bullying.  Bullying Report Form
 
Child Labor
Effective June 1, 2015, school administrators will no longer be issuing Iowa Child Labor Work Permits.  Youth, parents and employers complete a simplified form.  The form and instructions are available on the Iowa Division of Labor website.
 
Open Enrollment
According to the Iowa Department of Education, Open Enrollment is the process by which parents/guardians residing in an Iowa district may enroll their children into another Iowa school district. 
The deadline for students entering Grades 1-12 is March 1st. The deadline for students entering kindergarten is September 1st.

Homeschooling
If you wish to homeschool your student(s), there are two different homeschool options offered by the State of Iowa; Competent Private Instruction or Independent Private Instruction.  For more information on your private instruction options, see the Private Instruction Handbook

Competent Private Instruction (CPI): your student will have access to our homeschool assistance program, school devices, and curriculum, and the ability to dual enroll for both classes and activities.
Required form for CPI:  2024-2025 Form A
 
Independent Private Instruction (IPI):  If you choose IPI those options under CPI will not be available to you.

Family Educational Rights and Privacy Act (FERPA)

    The Family Educational Rights and Privacy Act (FERPA) (20 U.S.C. § 1232g; 34 CFR Part 99) is a Federal law that protects the     privacy of student education records. The law applies to all schools that receive funds under an applicable program of the U.S.     Department of Education.

    FERPA gives parents certain rights with respect to their children's education records. These rights transfer to the student when he     or she reaches the age of 18 or attends a school beyond the high school level. Students to whom the rights have transferred are     "eligible students."
